The services available to you will depend on your assessment, approval and individual support plan. We can help you understand the types of support that may be available and connect you with an independent aged care provider that may be suitable for your needs, preferences and circumstances.
Clinical care provides health and clinical support in your home when it is included in your approved support plan.

If you have already been assessed through My Aged Care, we can work with the information you provide to help identify providers that may be suitable.
If you have not yet been assessed, we can explain the next steps and where to obtain further information about the assessment process.
Based on the information you provide, we identify an independent aged care provider within our network that we believe may be a good fit for your needs, preferences and circumstances.
We can then help facilitate an introduction to that provider.
The aged care provider will discuss your individual needs, approved services, goals, preferences and any applicable costs or contributions with you.
You can involve your family, carer or representative in these discussions if you wish.
The provider will explain what they can offer and answer your questions before you decide whether you would like to proceed.
If you are comfortable proceeding, the provider will work with you to establish the appropriate arrangements for your care and services.
The provider will explain the relevant service agreement, services, costs and other arrangements before services commence.

Choosing aged care is an important decision. You should feel informed, respected and comfortable with the choices you make.
Our role is to help you understand your options and facilitate an introduction to an independent aged care provider that may be suitable for your needs.
You remain in control of whether you proceed.
We respect your privacy and handle your personal information in accordance with applicable Australian privacy requirements.
We will only share information with an aged care provider where appropriate and with your consent, unless disclosure is otherwise permitted or required by law.
You should always understand why your information is being collected, how it will be used and who it may be shared with.
The aged care providers we introduce are independent organisations.
Before deciding to proceed, you should have the opportunity to ask questions, understand the services available to you, discuss applicable costs and contributions, and consider whether the provider is right for you.
You are not required to accept an introduction or proceed with a provider simply because we have connected you.
Aged care providers are responsible for ensuring their workers meet applicable screening, suitability, training and other requirements before providing care.
If you have concerns about the conduct or suitability of a worker providing your care, you should raise the matter directly with your provider. You can also contact us if the concern relates to the introduction or support we have provided, and we can help you understand the appropriate next step.
You have the right to raise concerns or make a complaint about the services you receive.
Complaints can generally be raised directly with your aged care provider. You should not be disadvantaged or treated unfairly because you have raised a concern.
If you need further assistance, you can seek independent advocacy or contact the Aged Care Quality and Safety Commission.
You do not have to navigate the aged care system alone.
The Older Persons Advocacy Network (OPAN) provides free, confidential and independent advocacy support for older people.
An advocate can help you understand your rights, express your views and have your voice heard when making decisions about aged care.
You have the right to be treated with dignity and respect, to have your views and preferences considered, and to participate in decisions about your care.
