Care Homes on Wheels provides comprehensive, person-centred home care under the Australian Government’s Support at Home program.

We deliver services across all three funded categories:

Support is matched to your assessed needs, goals, preferences, language and cultural needs. The exact services available to you depend on your My Aged Care approval and support plan. We discuss these details with you during your free conversation.

Clinical Care

What it is

Professional health-related support delivered in the comfort of your own home by qualified care staff (such as registered nurses, allied health professionals and care workers).

Who it may help

Older people who need nursing care, help managing medications or health conditions, wound care, or allied health support to stay well at home.

What support can look like in everyday life

A nurse visiting to change a dressing, help with medications, monitor a chronic condition, or provide post-hospital support. A physiotherapist or occupational therapist working with you on exercises or practical strategies in your own home.

What is included

Services that fall under the Support at Home Clinical supports category. These are fully funded by the government, there is no participant contribution.

What is not included

Hospital treatment, services outside the approved Support at Home service list, or care that requires a different type of specialist setting.

  • Allied health services (such as physiotherapy or occupational therapy)

How often support may occur

As set out in your personalised support plan, from occasional visits to more regular scheduled care.

Yes, under Support at Home Clinical Care (fully government-funded). Confirm current rules with My Aged Care.

Referral, assessment or approval required?

Yes. A My Aged Care assessment and approval are needed for government-funded services.

Independence

What it is

Support that helps you stay active, confident and in control of daily life at home and in the community.

Who it may help

People who need assistance with personal care, getting out and about, short breaks for family carers, or equipment and modifications that make home life safer and easier.

What support can look like in everyday life

Help with showering, dressing or grooming so you start the day feeling comfortable. Transport to medical appointments or social activities. A few hours of respite so a family carer can rest. Guidance on simple aids or home changes that support independence.

What is included

Services approved under the Support at Home Independence category.

What is not included

Services outside your approved support plan or the official Support at Home service list.

  • Personal care (bathing, dressing, grooming, toileting)
  • Respite care for carers
  • Transport and community access
  • Social support and community engagement
  • Assistive technology recommendations and support
  • Home modification advice and support

How often support may occur

Flexible — according to your support plan and changing needs.

Can it be funded?

Yes, under Support at Home Independence. Contributions may apply based on your means assessment (note: from 1 October 2026 personal care is expected to have no out-of-pocket cost if approved — confirm with My Aged Care).

Referral, assessment or approval required?

Yes for government funding. A My Aged Care assessment is required.

Everyday Living

What it is

Practical help that keeps your home comfortable, clean and running smoothly so you can focus on the things that matter to you.

Who it may help

People who find housework, meal preparation, shopping or light home maintenance more difficult.

What support can look like in everyday life

Someone helping with vacuuming, laundry or changing the bed. Assistance preparing nutritious meals in your kitchen. Help with grocery shopping or simple errands. Light garden tidying to keep outdoor areas safe.

What is included

Services approved under the Support at Home Everyday Living category.

What is not included

Major renovations, heavy trade work, or services not on the approved Support at Home list.

Typical examples

  • Domestic assistance (cleaning, laundry, tidying)
  • Meal preparation and nutrition support
  • Shopping and errands
  • Home and garden maintenance (light tasks)

How often support may occur

As agreed in your support plan — weekly, fortnightly or as needed.

Can it be funded?

Yes, under Support at Home Everyday Living. Contributions usually apply based on your means assessment.

Referral, assessment or approval required?

Yes for government funding via My Aged Care assessment.
